A dental bridge fills a gap with one or more replacement teeth held by supports on either side or in selected configurations. It can be supported by natural teeth, adhesively retained by a wing, or carried by implants. Each design has different benefits, limitations and maintenance needs.
Conventional, adhesive and implant-supported bridges
A conventional bridge commonly uses crowns on supporting teeth. An adhesive or Maryland bridge uses a bonded wing and often requires less preparation, but is suitable only in certain positions and bites. An implant-supported bridge can replace several teeth without using neighbouring teeth as abutments but requires surgery and sufficient bone.
Choosing between a bridge and an implant
A bridge may be attractive when adjacent teeth already need crowns, when surgery is unsuitable or when timing favours a tooth-supported option. An implant avoids preparing healthy neighbouring teeth but has surgical, healing and maintenance considerations. Removable dentures and leaving the space are also alternatives in selected cases.
Assessment and treatment
The dentist examines the gap, supporting teeth, gums, bite and available space and uses X-rays where necessary. For a conventional bridge, abutment teeth are prepared and a temporary restoration may be fitted. The final bridge is checked for fit, contact, bite, appearance and cleansability before cementation.
Risks and expected maintenance
Supporting teeth can develop decay, pulp problems or fracture, and the bridge can loosen or chip. Adhesive bridges may debond. Implant-supported bridges carry implant-related risks. No design is maintenance-free, and failure of a support may affect the wider bridge.
How to clean under a bridge
Because connected replacement teeth cannot be flossed in the same way as separate teeth, patients need floss threaders, interdental brushes or other aids suited to the space. Cleaning at the gum line and beneath the replacement tooth helps control plaque and protect supports. The dental team should demonstrate the method.
